Note about collecting and holding personal information: This information is collected for the purpose of assessing your suitability for employment with CORE Education Limited. All information on this form is confidential and will be retained by CORE Education Ltd for six months unless you are appointed to a position or you specify otherwise. If your application is successful, this form will be retained on your personal file.

NB: If appointed, the permit must be sighted by CORE Education Ltd.

CORE Education Ltd requires you to disclose all convictions unless they are covered by the Criminal Records (Clean Slate) Act 2004. If you would like more information on what disclosure is required, please contact the HR Manager at CORE Education Ltd.
You must declare all of your convictions if you have:

• Been convicted of an offence within the last 7 years OR
• Been sentenced to a custodial sentence (e.g. imprisonment, corrective training, etc) OR
• Been ordered by the court during a criminal case to be detained in a hospital due to your mental condition instead of being sentenced OR
• Been convicted of a ‘special offence’ (e.g. sexual offending against children, young or mentally ill people).

In making this application, you consent to CORE Education Limited seeking verbal or written information about your suitability for the position from any of the referees you have nominated below. You authorise the information to be released to those involved in the selection process. The referees must include a current and at least one other recent employment-related referee. Generally personal referees will not be acceptable.

I declare that to the best of my knowledge the information provided in this application and in any resume/CV  enclosed is accurate and I understand that if any false or misleading information is given, or any material fact suppressed, I will not be employed, or if I am employed, my employment may be terminated.
